We open with young Johnny Blaze (Cage as a teen) in 1970s rural America. His father — a circus stunt rider — is dying of cancer. Johnny meets Mephistopheles / Mephisto (Peter Fonda) — the Devil — at a roadside diner. Mephisto offers Johnny a deal: Johnny's soul in exchange for healing his father's cancer.

Johnny accepts. His father is healed. Mephisto immediately takes his father in a roadside accident days later. Johnny realizes he has been deceived. Mephisto explains that the contract for his soul is now permanent — Mephisto can summon Johnny at any moment to become the Ghost Rider, a flame-headed cosmic enforcer.

Cut to: present day. Johnny Blaze (Nicolas Cage, now an adult) is a professional motorcycle stunt rider in Texas. He has been avoiding personal relationships and committed romantic involvement, fearing Mephisto will use his loved ones as leverage. He encounters his childhood girlfriend Roxanne (Eva Mendes) at a sporting event. They reconnect.

Mephisto summons Johnny. The transformation begins: his head transforms into a flame-skull, his motorcycle is replaced by a cosmic Hellcycle, and his powers include a hellfire chain whip. The transformation is involuntary — Johnny becomes Ghost Rider for periods that he cannot control.

The villain Blackheart (Wes Bentley) — Mephisto's son — has been attempting to take over Hell. He has been gathering 'fallen' souls from Earth to fuel his uprising. Mephisto sends Ghost Rider to stop him.

Ghost Rider fights Blackheart in the American Southwest. The final battle takes place in a small Western desert town. Ghost Rider defeats Blackheart by using his Penance Stare — a power that forces villains to experience all the pain they have caused. Blackheart is destroyed.
